Trauma-Related Symptoms

Trauma-related symptoms (subthreshold or partial PTSD) are meaningful post-trauma symptoms that fall short of full PTSD criteria but still cause real distress, defined per DSM-5-TR research conventions.

Symptoms

Symptoms from two or three of the four PTSD clusters -- intrusive memories, avoidance, negative mood/thinking changes, or hyperarousal -- following a traumatic exposure, lasting more than a month and causing distress or impairment, without meeting the full symptom count for PTSD.

Causes and risk

Exposure to a traumatic or highly stressful event; risk factors mirror those for full PTSD, per prevalence and burden research.

How it is evaluated

A formal clinical evaluation determines whether someone has partial versus full PTSD, since the symptom-cluster cutoff is a research convention rather than a fixed clinical rule.

Treatment

Trauma-focused psychotherapy (cognitive processing therapy, prolonged exposure, EMDR) is first-line, per both the 2023 VA/DoD guideline and the 2025 APA PTSD guideline, with sertraline, paroxetine, or venlafaxine as medication options and benzodiazepines specifically discouraged.

When to seek care

Partial PTSD carries genuine impairment and elevated suicidality risk -- it is not a benign condition. Any thoughts of self-harm should prompt immediate crisis support via 911 or the 988 Suicide and Crisis Lifeline.
